Wniosek złożony powyżej 21 dni po narodzinach dziecka upoważnia do 80% podstawy wymiaru. Dokumenty złożone w terminie upoważniają do 100% podstawy zasiłku macierzyńskiego i 60% podstawy wymiaru zasiłku rodzicielskiego — tj. 80% wynagrodzenia.
Jakie są terminy i procedury składania wniosków o płatny urlop rodzicielski i jakie dokumenty są wymagane?
Podstawą urlopu rodzicielskiego jest złożenie odpowiedniego wniosku do pracodawcy. W tym celu pracownik może:
- złożyć wniosek bezpośrednio po urlopie macierzyńskim — w terminie do 21 dni po porodzie. Dodatkowy, 9-tygodniowy okres rodzicielskiego płatny będzie w wysokości 70% podstawy,
- złożyć wniosek o udzielenie całości lub części urlopu rodzicielskiego – nie później niż 21 dni przed planowanym rozpoczęciem urlopu rodzicielskiego. W takich okolicznościach zasiłek macierzyński będzie wypłacany w wysokości 100% podstawy zasiłku — dokładnie przez 20 tygodni urlopu macierzyńskiego i 6 pierwszych tygodni urlopu rodzicielskiego.
